Position: Independent Living Program Manager
Department: Cascade Youth & Family Center (CYFC) – Independent Living Program (ILP)
Reports to: Community Programs Director, Vice President of Operations (VPO)
Position Summary
Provides independent living skills training to individuals and groups to assist the development and success of current and former foster youth. Process referrals, screen housing program candidates, community presentations, program reporting, budget development and recipe maintenances, and staff supervision as needed. This position is a full-time, 40 hours per week position with variable shifts including days, swing, weekends and holidays, although predominately days.
Duties
- Maintain professional contact with the Department of Human Services – Child Welfare District 10 as well as, various other agencies, and community members, as required to effectively facilitate referrals and promote successful case management
- Meet contract expectations regarding client engagement and productivity standards.
- Provide program orientation, needs assessments, transitional plans, transitional plan updates, housing program screening and general case management within the guidelines of ILP contract
- Provide effective program management in terms of monthly progress reports, case notes, electronic and paper file set up, program acceptance and termination, monitoring youth progress, monthly invoicing, database tracking, annual reports, and other tracking as needed
- Identify community resources and services available to assist youth throughout Jefferson, Crook and Deschutes Counties.
- Connect youth with community resources to support them with meeting their needs and goals.
- Plan and participate in monthly group activities and meetings. Coordinate with local organizations and business to provide experiential learning opportunities to groups that focus on independent living skills development, inter.
- Plan monthly/bi-monthly one-on-one meetings with youth to assist them in achieving their goals.
- Maintain regular communication with youth, outside of one-on-one meetings.
- Coordinate meetings with youth and support network as needed, in particular DHS Child Welfare Case Workers.
- Travel throughout Jefferson, Crook and Deschutes counties.
- Attend bi-annual ILP Provider Retreat, held at various locations around the state. Attend additional trainings and CYFC staff meetings as required.
- Attend annual Teen Retreat and Dream Conference. Transporting youth, chaperoning youth, and helping to facilitate event activities throughout the 4-day event.
- Provide thorough documentation and reporting.
- Maintain confidentiality regarding client and staff information.
- Supervise staff and youth
- Develop curriculum as needed to strengthen program and provide stronger youth services
- Assist and supervise staff members in providing quality program to all youth clients
- Budget development and monitoring
- Other duties and responsibilities as assigned
Qualifications
Knowledge, Skills and Ability: Ability to develop an environment where youth can succeed by ensuring the atmosphere and basic supplies for education, work, activities and similar are provided. Ability to effectively facilitate and document individual clients, independent living needs. Ability to interface effectively with other staff, referring agencies, parents, schools and others and maintain a professional demeanor at all times. Ability to provide crisis intervention and remain calm and in control during crisis situations. Ability to work independently and cooperatively as a team. Demonstrate strong written, verbal and interpersonal communication and organization skills. Strong database expertise.
Experience and Training: A bachelor’s degree with major study in social work or a closely allied field, or a minimum of 3 years of experience in the care and rehabilitation of youth. Experience with case management of at risk youth preferred. Valid Oregon driver license with clean driving record, and CPR/1st Aid certification required.
Physical Requirements: While performing the duties for the job, the employee is frequently required to stand and walk. The employee occasionally is required to use hands or finger, handle, or feel objects, tools or controls. The employee is occasionally required to sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus. While performing the duties of this position, the employee is regularly required to talk and hear. The noise level in the work environment is usually moderate.
