Position: Treatment Coordinator
Department: The Academy at Sisters
Reports to: Program Director
Position Summary
Provides daily coaching to youth and assists youth in understanding and implementing therapeutic goals and objectives as outlined in their individual treatment plan. The Treatment Coordinator serves as the primary contact for (a minimum of seven) youth and their families. The Treatment Coordinator will ensure that the daily needs of the youth and families are met. The Treatment Coordinator will act as liaison between the therapist, academic staff, referring professionals, youth and families. The treatment Coordinator will monitor youth progress and work in conjunction with therapist on transition planning for youth and families.
This is a full time, 40 hour per week, position. Hours may be flexible. Schedule is generally Wed-Sat, or Sun-Wed, and will also include additional hours as needed for Parent Workshops, Graduations and other important events.
Duties
- Provides daily supervision, coaching and support to youth in the program
- Ensures the needs of the youth and family are being met while in the program
- Responsible for intake of resident upon enrollment into the Academy
- Contributes to youth individual treatment plans and 90-day progress reports
- Reviews treatment plans and point cards with youth weekly or as determined by Treatment Team
- Schedules and conducts weekly phone calls/coaching sessions with youth, parents and families
- Meets weekly with Teacher and Therapist to discuss youth and documents on youth progress and challenges
- Attends weekly Treatment Team meetings and monthly staff meeting
- Reports on youth at weekly Treatment Team meetings using individual treatment plan and assists in identifying strategies to help youth advance through the program
- Co-creates and facilitates weekly psycho-educational and skill building groups for youth
- Coordinates, schedules and verifies youth participation at medical appointments
- Coordinates and schedules family, and family contracts
- Participates in the development and facilitation of Parent Workshops
- Assists Therapists and Teachers as an additional contact with all outside agency providers of youth as needed (Bend High School, adjunct Therapists, volunteer programs, etc.)
- Observe Equine therapy sessions as needed
- Maintains current and accurate documentation on youth in the program
- Assists with supervision of youth on activities
- Assists with additional support roles or duties
- Provides youth exit information to Program Director
- Assists with tours and promotion of the Academy at Sisters
- Assists with alumni outreach
- Act as emergency contact for Academy at Sisters staff
- Other duties as required by Program Director
Qualifications
Knowledge, Skills and Ability: Understanding of and ability to interact appropriately with at risk adolescent youth in a residential setting. Willingness to learn and implement the ABCR Program model. Willingness to work together with the Treatment Team and other staff to assist and support youth in their treatment and academic process. Ability to remain calm in crisis situations and provide a safe environment for all youth and staff. Ability to facilitate group and provide feedback to youth as needed. Excellent verbal and written communication skills. Ability to develop and maintain appropriate professional boundaries. Ability to work independently and cooperatively as a team. Ability to interface effectively with parents, referring professionals and others as needed. Maintains positive, professional demeanor and represents the agency in a profession manner at all times.
Experience and Training: Bachelor’s degree in Psychology, Sociology or related field and 2 years’ experience working with at-risk youth. Valid Oregon Driver License, current in CPR and First Aid.
Physical Requirements: While performing the duties for the job, the employee is frequently required to stand and walk. The employee occasionally is required to use hands or finger, handle, or feel objects, tools or controls. The employee is occasionally required to sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus. While performing the duties of this position, the employee is regularly required to talk and hear. The noise level in the work environment is usually moderate.
